In order for IFS to give you the accurate and timely service you deserve, we need to ensure that all the legal documents required by government entities are filled out and on file with IFS and those agencies.  This page is where you can download these documents. 

Once you have downloaded and completed the documents, please return them to IFS via email, or print them out and mail them to our main office.  Note that the download format for the documents listed on this page is PDF.
